The purpose of this report is to increase understanding of how the veteran utilizes the VA health care delivery system and how the system delivers care to the veteran. The majority of VA studies define and research one particular discharge or visit episode of care rather than a complete episode or series of episodes. In the present study, the care received and the resources consumed by a particular individual during one fiscal year are examined. A previous monograph (HSIS Report 82-2) studied the interactions of five different inpatient treatment programs by analyzing the number of individuals discharged from two or more of these programs in one fiscal year. This report looks at one of these inpatient settings, the VA hospital, and analyzes the phenomenon of multiple hospital discharges per person. One third of the individuals discharged from VA hospitals in FY 1981 had more than one discharge. The mean number of discharges per person was 1.55.

This is the 25th version of a bibliography on Post-Traumatic Disorder which was started by the author in May 1982. Most references cited are hard-cover monographs; about one-eighth are clinically relevant accounts of the war, some by veterans themselves. It includes citations based upon combat-related psychiatric disorders dating back to before WWI, but the references are predominantly related to the effects of the Vietnam War.

The purpose of this study was to determine the styles of nursing home care unit administration that were most effective in meeting the Veterans Administration mission, and the philosophy of nursing home care unit management that formed the basis for these practices. Work-related data were analyzed to develop a profile of the typical nursing home care unit supervisor in the VA nursing system. The results were to provide the basis for development of training programs for nursing home care unit supervisors. Results of the study showed 65% of respondents utilizing team nursing because of its adaptability to staffing resources, and 37% indicating that primary nursing care delivery ensured accountability and registered nurse responsibility for care planning. However, respondents reported disadvantages of both systems. For team nursing the disadvantage was reported as inability to provide or ensure continuity of care; insufficient staffs to meet workloads was reported as a disadvantage of the primary nursing delivery system.

Given the number of veterans whose claims are awaiting decisions, you asked that we evaluate VBAs implementation of the legislation. Specifically, you asked us to determine (1) what steps VBA has taken to implement the legislation and (2) how VBA is managing the slowdown in case processing.
